Doctor:

These are pictures of my hair, wet, dry, and of my chest and back (for
possible body hair donor areas).    Miscellaneous information:

Age:  23 (balding since 20 that I've noticed)

Medications:  Propecia, I've been on it for 3 consecutive months now, prior
to this, I was on it for 6, but then went off.

Goals:  I would like to increase density in the front and mid scalp to
eliminate the appearance of balding.  Specifically, I want to eliminate the
appearance of the small gap in the middle of my hairline, and have a
'normal' amount of coverage from the mid-scalp to the hairline.  I would also
like to take care of the bald spot at the back of my head, but that is
secondary.  Since I'm losing my hair at a relatively young age, I am very
interested in the possibility of using body hair as donor hair.  At this
time, lowering my hairline or filling in my temples is not of great
importance.

I am researching hair transplants for the possibility of receiving one in a
year's (or possibly two years') time.  Because of the odd fashion in which I
am losing hair (almost exclusively in the vertex area) I am not sure what
hair replacement options would be best for me, but I imagine that, left
untreated, I will be visibly bald sooner than were I losing hair in the
traditional PBM fashion.  I am still a student, so money is an option, and
if there are additional fees for using body hair in FUE rather than scalp
hair, I would appreciate being informed of them.

This is a very difficult situation. At 23 hair loss is a troublesome problem. It is one that could become quite severe over time. We do not have a crystal ball and cannot predict where you will end up.

I think that the best solution for you at this time is Propecia and other potential meds: Rogaine, Avodart, topical Spironolactone, Nizoral shampoo to mention a few.

You appear to have a natural part in the middle brought on by a cow-lick that results in the hair on the right side growing back toward your ear and the hair one the left growing in the opposite direction. This augments the early loss you are having in the frontal hair line area. The loss on the top is very early. The loss in the crown is also very early.

I do not recommend strip surgery to anyone under age 35 if they have not already had a strip. This is because it limits your hair style options later in life. You can sprinkle all the areas with the minimal number of grafts possible if you like, but I would prefer to treat you medically for one or two years and then reconsider surgical solutions. Grafting at this time could even accelerate your hair loss or cause shock loss. Therefore, try meds for now. This is your best solution at this time.
